Address Update Requested
The UHN is preparing to submit a comprehensive needs assessment to gather input from our communities. In order to ensure all tribal citizens have the opportunity to participate, we will be mailing the survey along with a long overdue newsletter. To make certain that you receive your copy, it is very important that all addresses be updated now. To update your address, please use the “membership update” form under Forms on the website. Be sure to turn it in to the enrollment office.
