Online Diabetes Registry Available

In 2010, UHN housholds self reported that 1 in 4 households has someone living with diabetes.  Our communities have very high rates of diabetes and the Tribe recognizes that those figures are growing.  In order to best help our citizens living with this condition, the UHN began a Diabetes Registry.

The Registry will allow the UHN to gather important data about the needs of our citizens living with diabetes.  Currently, being able to get UHN specific health data is very difficult as local providers do not track American Indian health data.  Therefore the only way to gather that information is to do it ourselves.  Additionally, being able to survey tribal citizens regarding their own needs helps us be more responsive in developing programs that YOU WANT AND NEED.

The registry is available for anyone who resides in a tribal household.  If you have not yet signed up and are living with diabetes, please sign up using the online web form below.
