Input Needed for 2015 Tribal Community Needs Assessment

For the last 15 years the United Houma Nation has been conducting community needs assessments every 5 years to gather tribal specific data that has proven to be invaluable in strengthening our grant writing and proposals to grow and develop further programs and services for our tribal citizens.  It's that time again and we need your help.

The 2015 UHN Needs Assessment is an online survey.  Who should complete the survey?

  • Enrolled tribal citizens ages 18 and over
  • Tribal citizens with their enrollment applications pending ages 18 and over

Please help ensure your friends and family members who do not have access to the online survey participate and ensure they are represented in the findings.  The UHN offices can assist by allowing individuals access to the internet to complete the survey.  Additionally, individuals can call (985)223-3093 to complete the survey by phone.

The survey includes many highly sensitive questions.  Please know your responses are confidential and no individual data will be shared.  The survey includes the following areas of focus:

  1. Household demographics, employment and educational data
  2. Family Health
  3. Diabetes Prevalence and Care
  4. Personal Wellness and Mental Health
  5. Deepwater Horizon Impacts

As an incentive to participate, the UHN is offering participants the opportunity to sign up for a $200 drawing, which will be conducted on Monday, June 1st.
